HEALTH BENEFITS BULLETIN
DOCTOR NOTES
Protect yourself from any employer complaints, 8.34 non availability or 70% by submitting doctor notes to PMA. (Class A and Class B must have notes stamped by PMA before submitting to the local). This does not guarantee the employer will not file a complaint against you. However, will help you justify your absence. Keep your original stamped notes in a safe place.
Return to Work (Class A/B):

Must obtain a Return to Work doctor’s note and submit to; PMA and the ILWU Local 13 -Dues Office (Must obtain Pacific Maritime Association’s stamp on doctor’s note before submitting to Local 13.).

It is the responsibility of each Class A/B individual to enroll in Dues Check Off again.

Class A/B individuals returning with restrictions must also submit a copy of the Return to work doctor’s note to the Health Benefits Department.
8.34 JPLRC CORRESPONDENCE
Any individuals that have received an 8.34 JPLRC Correspondence must obtain copies of all doctors’ notes and contact the ILWU Local 13, Labor Relations Representatives at (310) 830-1130; Monday through Friday, between the hours of 8:30 a.m. – 4:30 p.m.
VACATION/HOLIDAY
Please see the Records Clerk at the Dispatch Hall if you have any questions in regards to your vacation/holiday.
PLAN YEAR
The Plan Year is July 1 – June 30. This is the basis for all annual benefit renewals and limitations.
For example:

Chiropractic Treatment; limited to 40 visits per plan year

Skilled Nursing Facility; up to 100 days per plan year

UPDATING PERSONAL INFORMATION
REMINDER: It is your responsibility to update your personal information i.e. address, phone number, e-mail, beneficiary form(s), dependent add/delete or important notice regarding taxation, etc.
Important Note: All Updated Personal Information must be submitted by the Participant(s) only. (You become a Participant in the ILWU-PMA when you become registered.) You must notify PMA Payroll Office in person separately.

The ADA JPLRC meetings are scheduled on the 1st and 3rd Tuesday of each month at 9:00 a.m. at Pacific Maritime Association, 300 Oceangate, 12th Floor, Long Beach, CA 90802 (no parking validation – cash only).

The Health Benefits Officer maintains an open door policy, however, if you need special assistance, (i.e. retirement, disability or personal matter) we recommend that you schedule an appointment with Jerry Avila at (310) 830-6116.
