RETIREMENT
Members preparing to retire must pay any outstanding dues, fines and assessments by the time you retire. Be a Member in Good Standing.
200 HOURS PENSION CLAIM
Brothers and Sisters,
You may file up to 200 Hours Non-Industrial Pension Claim at any time. What is required: Doctor’s note on letter head, signed by the doctor, dates unable to work.

ILWU/PMA Benefit Plans in San Francisco (Administrator of the Plan) is required to keep all enrollees informed of any changes to our Welfare & Pension Plan. Do not be alarmed when you receive these mailers, if you have any questions, please call Benefit Plans @ (415) 673-8500.
CLARIFICATION OF MEMORANDUM OF UNDERSTANDING
A notice was mailed to clarify the recent mailing regarding Service Exclusions. Please note the following: NO MEMBERS (ACTIVES AND RETIREES) OR ANY MEMBER’S DEPENDENTS ARE LOSING THEIR COVERAGE THROUGH THE ILWU/PMA WELFARE PLAN. The Service Exclusion means that if your relative (who happens to be a doctor, chiropractor, or other provider of medical services) provides medical services to you or your dependents the cost of the medical services will not be covered by the ILWU/PMA Welfare Plan. This service exclusion does not apply to Dental and Vision Service Plan (VSP) providers of service. You may download a copy of this memo at www.benefitplans.com or contact ILWU/PMA Benefit Plans @ (415) 673-8500. Please read following examples of Service Exclusions:
Example 1:
Your spouse is a chiropractor. Chiropractic services provided to you or any of your dependents by your spouse will not be covered by the ILWU/PMA Welfare Plan.
Example 2:
Your son or daughter-in-law is a surgeon at a local hospital. Any surgical services your son or daughter-in-law may provide to you or any of your dependents will not be covered by the ILWU/PMA Welfare Plan. You or your dependents would be covered for services provided by other providers (who you are not related to) located at the hospital, but not for treatment provided by your son or daughter-in-law.
Example 3:
Your father is a dentist. Because this exclusion does not apply to the dental program, the dental services provided by your father will be covered.

Example 4:
Your daughter is a Vision Service Plan provider. Because this exclusion does not apply to the vision program, the vision services provided by your daughter will be covered.
UPDATING PERSONAL INFORMATION
REMINDER: It is your responsibility to update your personal information i.e. address, phone number, e-mail, beneficiary form(s), dependent add/delete or important notice regarding taxation, etc.
Important Note: All Updated Personal Information must be submitted by the Participant(s) only. (You become a Participant in the ILWU-PMA when you become registered.) You must notify PMA Payroll Office in person separately.
SAFETY TIP
Section 9 – DUTIES OF VEHICLE OPERATORS A vehicle operator shall carry out the following duties: Rule 901. Shall test brakes, shifting mechanisms, accelerator mechanisms, windshield wipers, steering gear, warning safety lights, horn and, where equipped, seat belts and mechanisms for raising, lowering and tilting before starting work. Upon completion of the inspection, the vehicle operator shall immediately report any defects to the foreman, walking boss or other employer representative on the job, who shall act promptly to correct the defect in the gear, vehicle, machinery or any unsafe working condition.
