CLASS āAā AVAILABILITY REQUIREMENT
SECTION 8.34 OF THE PCLCD
The Joint Port Labor Relations Committee has found that you have not submitted any documentation to them verifying your reason for not working in the industry from October 1, 2016 to October 31, 2016 and there is no documentation on file with Local 13 or PMA to excuse you for those months. If you have failed to work due to illness or injury, please submit any medical or disability documents verifying the period of your disability and if applicable, copies of State Disability Claim form, no later than Monday, January 16, 2017 to LRC Secretary, Taylor Agajanian at our Local 13 office at 630 S. Centre St., San Pedro, CA 90731.
You may also email your doctorās note to taylor@ilwu13.org or fax to (310) 830-3489 Attn: Taylor. Please include your work number on all documents. Failure to provide any documentation may result in your de-registration as a Class āAā Longshore worker.
