ATTENTION ALL CLASS B
If you are sick and/or had a work related injury and will be unable to work for 21 (twenty-one) or more consecutive days it is your obligation to submit the documentation as identified below:
- Pacific Maritime Association (PMA) local office to have stamped.
- ILWU Local 13 Dues office in order for your Dues and Worker Status to be adjusted in the database system.
Alternatively, if you are sick and/or had a work related injury and will be unable to work for less than 21 (twenty-one) consecutive days or have intermittent days off, it once again is your obligation to submit the documentation as identified below:
- Pacific Maritime Association (PMA) local office to have it stamped.
- Keep for your records.
Do not turn these notes into Local 13 Dues office.
Additionally, if you have been off due to an illness/injury and must return with job restrictions, it is your obligation to submit the documentation as identified below:
- Pacific Maritime Association (PMA) local office to have it stamped.
- Bring the stamped note to the Health Benefits office at Local 13.
- The Health Benefits office will give you further instruction as it relates to notifying the Dues office at Local 13.
