ANNUAL OTHER INSURANCE COVERAGE
ILWU-PMA Coastwise Indemnity Plan Participants:
The Plan is requiring completed Other Insurance Coverage Forms on an annual basis for those members that have covered dependents. Each October you will receive notification requesting the completion of the Other Insurance Coverage (OIC) form. Completing this annual requirement will make it simpler to expedite claim benefit payments for you and your dependents and avoid requests based on individual claim denials for each family member throughout the year. Even if you have provided this information earlier this year, you will need to submit the completed enclosed form to avoid any delay in your family’s claim benefit payments.
Therefore, you must:
- Read, complete, sign and return the Other Insurance Coverage Form by the required date. You may fax the completed form to (415) 749-1400 or mail using the pre-paid addressed envelope provided by ILWU-PMA Benefit Plans.
If you do not return the form:
- Your dependent(s) benefit claim(s) for dates of service after December 31, 2017 will be denied until the form is received.

AMERICAN CANCER SOCIETY GUIDELINES
All Participants and Beneficiaries Enrolled in the ILWU-PMA Coastwise Indemnity Plan:
This is a reminder that coverage for routine mammograms, Pap smears, Prostate Specific Antigen (PSA) tests, and related office visits are payable according to the American Cancer Society guidelines in effect at the time of treatment. See the ILWU-PMA Coastwise Indemnity Plan Supplement Summary Plan Description on page 29, which is also available online at:

HONORING VETERANS
It’s the one day set aside each November to honor those who have served and continue to serve our nation. The holiday also offers a great opportunity to teach civilians about the sacrifice service members and their families make year-round. Here are some ideas to acknowledge this holiday:
At home:
- Wear a red poppy or yellow ribbon to show support for veteran and active duty service members
At work:
- Take time out of the day to acknowledge veterans in your workplace.
