HEALTH BENEFITS BULLETIN
WELFARE CLAIMS
Brothers and Sisters,
If you didn’t make the 800 hours required for Welfare in 2016 due to a work or non-work related injury or illness
Now would be the time to gather your doctor(s) note (on doctor’s letter head, currently dated and signed by your doctor) stating that you were a patient, under his/her care an unable to work from (date) to (date).
During the month of March or April the ILWU/PMA Benefit Plans, the Administrators of the plan will mail out letters to those losing benefits as of July 1, 2017.
200 HOURS/LS208/PENSION/CLAIMS
You may file up to a maximum of 200 hours claim if, you have a non-work related illness or injury. What is required to file a claim: A doctor(s) note (on doctor’s letter head, currently dated and signed by your doctor) stating that you were a patient, under his/her care and unable to work from (date) to (date).
For a work related injury, you must submit an updated LS208. You may obtain this form from your Workers’ Compensation Attorney/Adjustor or the US Department of Labor.
All claims must be filed at Local 13, Health Benefits Department. Please inform the staff what claims you’d like to file.
VACATION/HOLIDAY/PAY SHORTAGES CLAIMS
Please see or call Records Clerk at Local 13 Dispatch Hall, (310) 834-7386 with any questions or concerns in regards to your Vacation, Holiday or Pay Shortages.

Updating your personal information; marriage status, dependents, new address & phone number, updating beneficiaries: Any changes must be submitted to Local 13, by the Participant(s) only (you become a Participant in the ILWU-PMA when you become registered).
