NOTICE TO COASTWISE INDEMNITY PLAN PARTICIPANTS
A notice to Coastwise Indemnity Plan Participants has been mailed regarding the reinstatement of Francisco Javier Correa, M.D. effective October 1, 2015.
By notice dated May 8, 2015, you were informed that the ILWU-PMA Welfare Plan (“Plan”) would no longer reimburse Plan participants for services, supplies, tests, or prescriptions provided by eight named providers, including Francisco Javier Correa, M.D. Please take notice that the Trustees of the Plan, who have sole authority to administer the Plan, have determined that the Plan will again reimburse participants and beneficiaries of services provided to them by Dr. Correa on or after October 1, 2015. No other changes have been made to the notice of May 8, 2015.
A complete and current list of all providers and health care facilities whose services, supplies, tests and prescriptions the Plan does not reimburse is posted on the Plan’s website, www.benefitplans.org. Please consult the website or telephone the Benefit Plans Office at (415) 673-8500 to get the most current information regarding providers and facilities whose invoices will not be reimbursed.
UPDATING PERSONAL INFORMATION
REMINDER: It is your responsibility to update your personal information i.e. address, phone number, e-mail, beneficiary form(s), dependent add/delete or important notice regarding taxation, etc.
Important Note: All Updated Personal Information must be submitted by the Participant(s) only. (You become a Participant in the ILWU-PMA when you become registered.) You must notify PMA Payroll Office in person separately.

HEALTH TIP
Preventing the Flu: Good Health Habits Can Help Stop Germs. The single best way to prevent seasonal flu is to get vaccinated each year, but good health habits like covering your cough and washing your hands often can help stop the spread of germs and prevent respiratory illnesses like the flu. There are also flu antiviral drugs that can be used to treat and prevent flu. Remember to avoid close contact with people who are sick, stay home when you are sick.
SAFETY TIP
Section 2 – Rule 237. A ladder shall be provided in all holds where employees are engaged in operations. Where there are two units or gangs operating in a hatch, there shall be two ladders provided.
ADA ACCOMMODATIONS
If you have any changes to your ADA accommodations you must attend the ADA JPLRC.
ADA JPLRC meetings are scheduled the 1st and 3rd Tuesday of each month at 9:00 a.m. at Pacific Maritime Association, 300 Oceangate, 12th Floor, Long Beach, CA 90802 (no parking validation).
