Doctors’ notes are required for the following:
Vacation Claims – all claims are filed with the Records Clerk at the Dispatch Hall
Class A – 8.34 availability
Class B – 70% availability
Sick Dues – submit to Dues Office
Filing up to 200 hrs./Pension Claim (non-industrial)
LS208 – Filing Industrial Pension Claim
Termination of Welfare – Once you are notified by the Plan Administrators (ILWU/PMA Benefit Plans Office)
ALL NOTES MUST BE STAMPED BY PMA BEFORE SUBMITTING TO THE LOCAL

With the addition of this new Roth 401(k) feature, you will have another choice on how to save for your retirement – which includes the potential to earn income tax-free growth and tax-free withdrawals (provided certain requirements are met)*.

*A distribution from a Roth 401(k) is tax free and penalty free, provided the five-year aging requirement has been satisfied and one of the following conditions is met: age 59 ½, disability, or death.

REMINDER: It is your responsibility to update your personal information i.e. address, phone number, e-mail, beneficiary form(s), dependent add/delete or important notice regarding taxation, etc.
Important Note: All Updated Personal Information must be submitted by the Participant(s) only. (You become a Participant in the ILWU-PMA when you become registered.) You must notify PMA Payroll Office in person separately.
